Recordbook Forms

Recordbook To Do List


• Recordbook forms must be signed by a Club Manager or a Project Leader before they are turned in. UNSIGNED forms in your recordbook will not be accepted. Project forms require a leader’s signature.

4-H Member Recognition Application form must be clipped to the outside front cover of your Recordbook or attached to the front of the project form. The 4-H Member Recognition Application form must be signed by your Club Manager or it will NOT be accepted. A member receives a year pin by submitting a project form only. A member receives a year pin and a project medal when the completed Recordbook is submitted. If you need a copy of the Member Recognition Application contact your Recordbook Leader.

• A 3" x 5" information card must be completed and stapled to the outside center top front of each Recordbook cover. Project forms do not require the 3" x 5" card. The 3" x 5" information cards are available from our office.

• Complete the top portion of the Recordbook Check Sheet (beige). Clip it to the inside of the front cover of your Recordbook. Project forms do not require this sheet. (Our District is 10). If you need a copy of this form contact your Recordbook Leader.
